We welcome you to our practice where we are improving smiles everyday with the latest in advanced technology including Invisalign, Ormco Ice and mini diamond brackets as well as other custom appliances. This letter is enclosed to help you become familiar with the procedures that are typical of orthodontic treatment in our office.

This is what you can expect on your first visit for the INITIAL EXAMINATION:

1. Assistance in completing our health history form
2. A private conference with Dr. Cohen
3. A thorough visual examination of the mouth by the doctor
4. An orientation tour of our office

This first visit is designed to help you understand the orthodontic status of the month and the effect it has on health and appearance. As a courtesy to you, there is no fee for this examination.

If immediate treatment is indicated, as a convenience for you, our office offers the opportunity to get treatment started with diagnostic records. This will consist of:

1. Impressions for study models
2. Photographs within the mouth and of the face
3. A complete series of x-rays

Once all of our diagnostic records are made, we will discuss:

1. Dr. Cohen’s findings derived from the analysis of the records
2. The treatment plan and expected results
3. The length of treatment and the fee
4. Financial arrangements and policy regarding insurance coverage, if applicable

If you have orthodontic insurance, please bring a completed form with you to the INITIAL visit.

The next step is a quick 15-minute appointment to place “spacers” in the mouth. These will create the space needed to band the molars. This visit will be followed a week later by one or two longer appointments to place the orthodontic appliances.
